Sign-Up for Service
Follow
the links to the appropriate form below to sign-up for the telephone
service you need. After completing the form, mail it to the address
above or fax it to 503 972 0399. A representative from Douglas Telecom
will contact you shortly after receipt of your sign-up form.
Choose
one of the following telephone service options:
1.
Standard Local Telephone Service -
Low
monthly fee of only $15.00 per line, with no additional fees or surcharges
for the local service (see sign-up form for explanations and details).
If there is no long-distance usage within a given month, the total
bill for the month will only be $15.00 for the local VoIP
telephone service. All local accounts include caller ID, call
waiting, call forwarding, real-time account management and
statistics, as well as voicemail. Voicemail can be accessed either
through the line or downloaded to your computer for review.
2.
Flexible Local Telephone Service
- Monthly fee of only $12.50 per line, with 500 free minutes each
month for calls to local, domestic long distance and select
international destinations (see sign-up form for explanations and details).
Incoming calls are free. All local accounts include caller ID, call
waiting, call forwarding, real-time account management and
statistics, as well as voicemail. Voicemail can be accessed either
through the line or downloaded to your computer for review.
3.
Basic Local Telephone Service -
Low
monthly fee of only $5.00, with all domestic and several
international destinations billed at 5¢ per minute (see sign-up form for explanations and details).
If there is no outgoing usage within a given month, the total
bill for the month will only be $5.00 per line. Incoming calls are
free. All local accounts include caller ID, call
waiting, call forwarding, real-time account management and
statistics, as well as voicemail. Voicemail can be accessed either
through the line or downloaded to your computer for review.
4.
Unlimited Local Telephone Service
- Costs only $24.95 per line per month, with free calls to
domestic and select
international destinations (see sign-up form for explanations and details). All local accounts include caller ID, call
waiting, call forwarding, real-time account management and
statistics, as well as voicemail. Voicemail can be accessed either
through the line or downloaded to your computer for review.
5.
Sign-up
for call-forwarding service. You will be issued a telephone
number from Douglas Telecom from which you can forward calls to
any telephone number desired. Call-forwarding settings can be
changed at will through your online account. Each local telephone
number has a low monthly fee of only $15.00, with no additional fees
or surcharges to the local service (see sign form for explanations and
details). If there is no long-distance usage within a given month, the
total bill for the month will only be $15.00 for the local VoIP
telephone service. Long distance charges apply for calls forwarded to
numbers outside of the local service area -- see form for details.
6.
Sign-up
for 1(800) number telephone service with optional purchase of
VoIP router. 1(800) number can be issued as part of a full VoIP
telephone service package, or calls can be forwarded from the
1(800) number to a telephone number of your choice. Each toll-free
number has a monthly fee of $5.00. Each toll-free account features
real-time account management and access to real-time call statistics.
Toll free number settings can be viewed and changed in real-time via
your online DTI account.
7.
Sign up for long distance only service
with optional purchase of VoIP router. Each outgoing only VoIP long
distance line has a monthly fee of only $2.50. Each account features
real-time account management and access to real-time call statistics
via your online DTI account.
